To view a consent form for an existing patient, first search for that patient using their surname, NHS number or hospital number.
Tap on your patient, then select “View Patient”.
Now you can see the patient’s information and all historical consent forms that are saved in the Vault system. Tap on the row for the relevant procedure and then tap “View Consent Form”.
